For Patients

Patients seeking to discuss exam results should contact their referring physican's office directly. Our Radiologists review the images and report the findings to your physician, who will then discuss the results directly with you.

To request your images and report:
A CD with images/report will be provided as requested at no charge. Please fill the Release of Information form (pdf), sign it, and fax it to the appropriate Customer Service Center where you had your exam done.

Please include the following information:

Patient Name

Birthdate

Medical Record Number (if you know it)

Your contact telephone number

Date and type of exam requested

Complete address including zip code

Radiology Consultation Service:
Radiology Consultation Service provides official reading for exams performed at another facility. A requisition signed by the provider must be submitted with CD or Films. The following information must be included.

Patient's name

Medical Record Number

Date of exam requested for second reading

Type of exam requested for second reading

Diagnosis Provider signature

Contact telephone number

Type of insurance with appropriate authorization number if required. If no authorization is needed, please indicate "No Authorization needed"

For Referring Practices

To speak with one of our radiologists:
Our radiologists are available to speak with referring providers. Call the Radiology Customer Service Center at (415) 353-1640 and someone will be happy to assist you. Please have your patient's name available.

Community Physicians: accessing my patient results
Patient report is mailed to you within 3-5 days from being completed. If you prefer to have the reports sent sooner to you by fax, please call the Radiology Customer Service at (415) 353-1640.

Community physicians can access results by same-day messenger or overnight delivery. To request a hard copy report and CD of images, please call the Radiology Customer Service Center at (415) 353-1640.

UCSF physicians: accessing my patient results
In person:
We encourage all UCSF referring physicians to visit our reading room to view images and consult with a radiologist. Please contact the Radiology customer service center with any questions.

Online:
Images may be viewed online from your office if you have access to UCSF's WebPACS system.

If you are a UCSF provider and would like to open a WebPACS account, please download this form (pdf) and fax it to (415) 502-1837.
